I’m supposed to take an exam but I am sick, what should I do?

It is assumed that if you take an exam you are fit to sit. If you are not well enough to take an exam, you should not attend the exam and then submit a PMC. Guidance on submitting PMCs is here and the Students Union advice center can also help you.
